Who do we help?
We support:
- war veterans and service members undergoing treatment or rehabilitation in medical institutions in Lviv;
- families of military personnel — including spouses, children, and parents who need psychological or social support.
How do we help?
Dzherelo Centre specialists provide a wide range of social services, including:
- representation of interests (assistance with documents, registration, etc.);
- social accompaniment — ongoing support in difficult life circumstances;
- emergency (crisis) intervention;
- social adaptation (training, skill development, participation in self-help groups);
- integration and reintegration (restoration of social and family ties);
- mediation, counseling, and information (for conflict resolution and stress reduction).
We work not only with documents — we build trust and support the path to recovery.
How to access the services?
To receive services, you need to prepare:
- an application (completed at the social work department);
- copies of passport pages 1, 2, and 11;
- tax identification number (TIN);
- proof of residence;
- medical-social expert commission certificate (MSEC) or pension certificate (if available);
- IDP certificate (if available);
- needs assessment report (prepared by our specialist);
- child’s birth certificate (if applicable);
- income declaration (if necessary);
- other documents depending on the situation.
