
At Dzherelo Centre, we support children and youth with disabilities, creating a safe, supportive environment where they can grow, communicate, learn life skills, and enjoy each day with dignity and joy.
Our day care service is more than supervision — it’s about building meaningful relationships, discovering talents, and helping each young person feel seen, understood, and included.
Who do we support?
The day care service is intended for young people with disabilities aged 18 to 45 years who:
- require daily assistance;
- are not fully independent in everyday life;
- face difficult life circumstances due to their disability.
All services are provided free of charge.
Services are for the residents of the Lviv community.
How do we help?
Young people can come to the Centre during the day to connect with others, stay active, and engage in meaningful activities. We do not provide accommodation, but every day at Dzherelo offers a warm, understanding environment.
We help participants to:
- develop everyday life and self-care skills;
- strengthen their self-confidence;
- maintain physical activity and emotional well-being.
Throughout the day, participants engage in creative projects, movement activities, social interaction, and rest — all under the guidance of professionals. We also provide a daily meal and, if needed, organize transportation to and from the Centre.

How to access the services?
To use the day care service, submit your documents to your district Social Protection Department. If places are available, you will be gladly included in the program.
What you need to prepare:
-Application (filled out at the Social Protection Department)
-Passport of the person with a disability and their representative (pages 1, 2, 11)
-Tax ID number (ІПН)
-MSEC certificate or summary of functional assessment
-Pension certificate
-Court decision on limited legal capacity (if applicable)
-Guardianship/custody documents (if applicable)
-Needs assessment report (if available)
-IDP registration certificate (for internally displaced persons)
-Medical summary (form 027/o)
