
At Dzherelo Centre, we support seniors with care and respect. Our help goes beyond daily tasks — we work to preserve activity, social connection, and meaning in life.
For some, this means home care; for others — joining interest clubs, learning new skills, engaging in creative activities, and meeting new people. Our mission is to be there for each person and make everyday life warmer, easier, and more fulfilling.
Who do we support?
We provide support to seniors who:
- need help with everyday household tasks;
- wish to stay active and socially connected;
- are looking for new opportunities to communicate and grow.
How do we help?
Home Care
Our social workers visit clients at home and assist with:
- personal hygiene, medication, and nutrition;
- household tasks like cooking, cleaning, and shopping;
- safe mobility around the home;
- paperwork and utility payments;
- connecting with medical, social, and other services;
- emotional support and friendly conversations.
Note: This service is not available for individuals requiring 24-hour care.

Social Adaptation
This service helps seniors remain active and involved. We support them to:
- better understand their life situation and find ways to improve it;
- maintain and build social ties;
- join clubs, workshops, and learning opportunities.
Seniors take part in choir singing, dance classes, and hobby groups — exploring new interests, socializing, and expressing themselves.
How to access the services?
For Home Care:
Required documents:
- application (submitted by the individual or a legal representative);
- medical certificate on self-care ability;
- copies of passport and tax ID (IPN);
- MSEC certificate (if applicable);
- guardian’s documents (if needed);
- IDP certificate (if applicable);
- needs assessment form (if available);
- other documents based on the situation.
For Social Adaptation:
Required documents:
- application (submitted by the individual or a legal representative);
- medical certificate;
- copies of passport and tax ID (IPN);
- documents confirming status, benefits, or residency;
- representative’s documents (if applicable);
- needs assessment form (if available);
- other documents if necessary.
Our locations
Social adaptation and active leisure activities take place at various sites in Lviv:
- Day Centre at S. Petliury Street
- Molodvizh Hub at M. Skoryka Street
- Palace of Youth at Yevhena Petrushevycha Square
- Library at Kniazya Romana Street
- Library at Okunevskoho Street
Dance classes are held at Ohiienka Street, and choir practice at Ruska Street.
Home care services are provided at the client’s place of residence.
