Children at risk and/or with disabilities and their families

At Dzherelo Centre, we support children with disabilities and children at risk — as well as their families — every day. We understand how many challenges you face, which is why we offer programs that help children develop, restore family resources, and build a life with dignity and quality.

Our services include:

Early intervention for the youngest children (from birth to 4 years old);
Comprehensive rehabilitation (ages 4 to 18);
Day care for children (ages 4 to 18).

All our programs are tailored to the individual needs of each child. We want every child to have the opportunity to learn, communicate, make friends, and simply be happy.

Who do we support?

You can contact us if:

  • you are a parent of a child at risk or one who is experiencing developmental difficulties;
  • your child has been officially recognized as having a disability;
  • you have concerns about the development of your child (from birth to 4 years old).

How do we help?

Early intervention (for children from birth to 4 years old)

This service is for families with children who have developmental difficulties, are at risk of such difficulties, or have been diagnosed with a disability.

Our team helps:

  • notice the first developmental signals;
  • develop a support strategy tailored to your child;
  • teach parents how to implement these strategies in everyday life.

We don’t just work with the child — we work with you. Because real progress happens in familiar environments — at home, during play, in communication.

Our interdisciplinary team includes:

  • speech therapist
  • physical therapist
  • occupational therapist
  • psychologist
  • doctor (pediatric neurologist)
  • social work specialist

You can also assess your child’s development yourself using our mobile app .

Comprehensive Rehabilitation (for children with disabilities, ages 4–18)

This service helps children with disabilities function better in everyday life — at school, at home, and with peers.

Our specialists develop an individual rehabilitation plan, based on the needs of the child and family — always considering the child’s capabilities, the family’s rhythm, and life’s realities.

The program can be taken once every 6 months:
-Intensively (10 consecutive days), or
-At a more relaxed pace, according to a flexible schedule.

The team includes a physical rehabilitation therapist, occupational therapist, speech therapist, psychologist, social worker, and doctor.

Day Care (for children with disabilities, ages 4–18)

We provide a space where children can spend their day in a warm, friendly atmosphere, communicate, develop, and relax.

This is more than just care — it’s about daily interaction, support, and small victories.

Children benefit from:

  • sessions with specialists;
  • social interaction with peers;
  • leisure and creative activities;
  • one meal a day.

Each branch team includes a manager, methodologist-educator, social educator for children with disabilities and their assistant, and a social worker.

This service is available to families registered in the Lviv community.

How to access the service?

We understand that collecting documents can be a hassle. That’s why we’ve outlined everything as clearly as possible — so your child can easily access support at Dzherelo Centre.

How to Apply for Early Intervention (ages 0–4)

Please prepare the following documents:

  • proof of the child’s place of residence
  • copy of one parent’s passport (pages 1, 2, and 11)
  • child’s birth certificate
  • child’s tax identification number
  • medical summary (form 027/o)
  • disability certificate (form 080/o), if available
  • individual rehabilitation plan, if available
  • doctor’s referral (if the child does not have a disability)
  • guardianship documents (if applicable)

Write an application (can be filled out on-site at the Centre).

Submit the documents to the Dzherelo Centre.

How to Access Comprehensive Rehabilitation (ages 4–18)

Prepare the following:

  • application (completed at the Department of Social Protection)
  • certificate of family composition
  • child’s birth certificate
  • copy of one parent’s passport
  • child’s tax identification number
  • medical summary (form 027/o)
  • guardianship documents (if needed)

How to Access Day Care (ages 4–18)

*Subject to availability of places.

Prepare the following documents:

  • application (completed at the Department of Social Protection)
  • child’s birth certificate or passport
  • copy of one parent’s passport
  • child’s tax identification number
  • medical summary (form 027/o)
  • disability certificate or LKK medical commission conclusion
  • medical certificate №080-3/o (if available)
  • guardianship documents (if needed)
  • needs assessment report (if already completed)

Submit your documents to your local Department of Social Protection.
