Our mission is to be there for each person and make everyday life warmer, easier, and more fulfilling.<\/p>\n\n\n\n<div style=\"height:40px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Who do we support?<\/h2>\n\n\n\n<p>We provide support to seniors who:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>need help with everyday household tasks;<\/li>\n\n\n\n<li>wish to stay active and socially connected;<\/li>\n\n\n\n<li>are looking for new opportunities to communicate and grow.<\/li>\n<\/ul>\n\n\n\n<div style=\"height:39px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">How do we help?<\/h2>\n\n\n\n<p><strong>Home Care<\/strong><br>Our social workers visit clients at home and assist with:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>personal hygiene, medication, and nutrition;<\/li>\n\n\n\n<li>household tasks like cooking, cleaning, and shopping;<\/li>\n\n\n\n<li>safe mobility around the home;<\/li>\n\n\n\n<li>paperwork and utility payments;<\/li>\n\n\n\n<li>connecting with medical, social, and other services;<\/li>\n\n\n\n<li>emotional support and friendly conversations.<\/li>\n<\/ul>\n\n\n\n<p>Note: This service is not available for individuals requiring 24-hour care.<\/p>\n\n\n\n<figure class=\"wp-block-image size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"790\" height=\"444\" src=\"https:\/\/dzherelo.org.ua\/wp-content\/uploads\/2024\/12\/novyj-format-na-sajt-45-1.png\" alt=\"\" class=\"wp-image-2102\" srcset=\"https:\/\/dzherelo.org.ua\/wp-content\/uploads\/2024\/12\/novyj-format-na-sajt-45-1.png 790w, https:\/\/dzherelo.org.ua\/wp-content\/uploads\/2024\/12\/novyj-format-na-sajt-45-1-300x169.png 300w, https:\/\/dzherelo.org.ua\/wp-content\/uploads\/2024\/12\/novyj-format-na-sajt-45-1-768x432.png 768w\" sizes=\"auto, (max-width: 790px) 100vw, 790px\" \/><\/figure>\n\n\n\n<p><strong>Social Adaptation<\/strong><br>This service helps seniors remain active and involved. We support them to:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>better understand their life situation and find ways to improve it;<\/li>\n\n\n\n<li>maintain and build social ties;<\/li>\n\n\n\n<li>join clubs, workshops, and learning opportunities.<\/li>\n<\/ul>\n\n\n\n<p>Seniors take part in choir singing, dance classes, and hobby groups \u2014 exploring new interests, socializing, and expressing themselves.<\/p>\n\n\n\n<div style=\"height:40px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">How to access the services?<\/h2>\n\n\n\n<p>For Home Care:<br>Required documents:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>application (submitted by the individual or a legal representative);<\/li>\n\n\n\n<li>medical certificate on self-care ability;<\/li>\n\n\n\n<li>copies of passport and tax ID (IPN);<\/li>\n\n\n\n<li>MSEC certificate (if applicable);<\/li>\n\n\n\n<li>guardian\u2019s documents (if needed);<\/li>\n\n\n\n<li>IDP certificate (if applicable);<\/li>\n\n\n\n<li>needs assessment form (if available);<\/li>\n\n\n\n<li>other documents based on the situation.<\/li>\n<\/ul>\n\n\n\n<p>For Social Adaptation:<br>Required documents:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>application (submitted by the individual or a legal representative);<\/li>\n\n\n\n<li>medical certificate on self-care ability;<\/li>\n\n\n\n<li>copies of passport and tax ID (IPN);<\/li>\n\n\n\n<li>documents confirming status, benefits, or residency;<\/li>\n\n\n\n<li>representative&#8217;s documents (if applicable);<\/li>\n\n\n\n<li>needs assessment form (if available);<\/li>\n\n\n\n<li>other documents if necessary.<\/li>\n<\/ul>\n\n\n\n<div style=\"height:40px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">Our locations <\/h2>\n\n\n\n<p>Social adaptation and active leisure activities take place at various sites in Lviv:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Day Centre at S. Petliury Street<\/li>\n\n\n\n<li>Molodvizh Hub at M. Skoryka Street<\/li>\n\n\n\n<li>Palace of Youth at Yevhena Petrushevycha Square<\/li>\n\n\n\n<li>Library at Kniazya Romana Street<\/li>\n\n\n\n<li>Library at Okunevskoho Street<\/li>\n<\/ul>\n\n\n\n<p>Dance classes are held at Ohiienka Street, and choir practice at Ruska Street.<\/p>\n\n\n\n<p>Home care services are provided at the client&#8217;s place of residence.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>At Dzherelo Centre, we support seniors with care and respect.
